Step One:Teacher Self-Assessment Principal Self-Assessment

Step Two:SMART GOALSaligned with District Goals IPDP Help Sheet

IPDP Form Updated 3/24/2010

Step Four: Submit IPDP see Organizing for High Quality Professional Development (Rubrics in back)

Step Five: Professional Development Options 6 Credit hours from an Accredited Institution OR 18 CEU's or EOA's Pre-approved Providers List (all others check with state or see your RSLPDC Rep) Equivalent Other Activity (EOA)  is a way to earn CEU's independently through job embedded professional development. EOA Proposal Form(consult EOA Guideline Chart prior to filling out form for review) EOA Guideline Chart IPDP/EOA Activity Form (use this to record your activities once your proposal is approved)

Step Six:Fill out Licensure Checklist& 5 Year Renewal Application or other applications  When submitting your application for review include the following: Copy of your IPDP Copy of your current teaching license Licensure Checklist Transcripts, CEU Certificates or EOA Verification Forms Completed Application
